Start with the front side of pentagon facing up.

 

Prepare to convert all the creases into either valley-folded or mountain-folded ones as shown on the left crease pattern (CP).

Complete the conversion of the CP given on the step #1.

 

Prepare to press the model to make it flat by following the arrows shown on the left picture.

FORGET ME NOT



There is a little flower
With a name that mean's a lot,
It is the prettiest little bloom,
It; s the forget me not.

They are such pretty little blooms
Their petals so small and blue,
And as i gently tend to them
I always think of you.

I think of how you tended them
And how you watched them grow,
As you walked around your garden
Which you loved so much, i know.

It is a lovely flower.
It does mean such a lot,
And as it's name already say's
I will forget you not.
